Telehealth Consent
What Is Telehealth?
Telehealth refers to the delivery of healthcare services using electronic communications and technology, allowing patients to connect with licensed healthcare professionals remotely — from home, work, or wherever you are.
Through Bahari Medical's partner platforms, you can access virtual primary care, urgent care, behavioral health, and dermatology services without traveling to an in-person facility.
Effective Date: 2025
Benefits of Telehealth
- Convenient access to care from any location with an internet connection
- Reduced travel time and waiting room delays
- Access to licensed providers across the country
- Flexible scheduling for non-emergency health needs
- Continuity of care for ongoing health management
Limitations and Risks
Telehealth has limitations compared to in-person care. You should be aware of the following:
- Providers cannot perform physical examinations through telehealth visits.
- Some conditions require in-person evaluation, testing, or treatment that cannot be delivered virtually.
- Technical difficulties (poor connection, equipment failure) may interrupt a visit.
- In rare cases, information transmitted may be insufficient to make a clinical determination, and an in-person visit may be recommended.
- Telehealth services are not available in all states and may vary by location.
Not for Emergencies
Telehealth is not appropriate for medical emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
Do not use telehealth services for chest pain, difficulty breathing, stroke symptoms, severe bleeding, loss of consciousness, or any other life-threatening condition.
Your Consent to Telehealth
By enrolling in a Bahari Medical care plan and accessing telehealth services through our partner platforms, you acknowledge and agree to the following:
- You understand the nature, benefits, and limitations of telehealth services as described above.
- You consent to receiving healthcare services through electronic communications.
- You understand that no provider-patient relationship is established through the Bahari Medical website itself — that relationship is formed on the partner platform at the time of your visit.
- You have the right to withdraw consent for telehealth services at any time without affecting your right to future care.
- You understand that telehealth services are not a substitute for emergency care or comprehensive in-person medical evaluation when clinically indicated.
Prescriptions
Prescriptions may be issued when medically appropriate following evaluation by a licensed healthcare professional and where permitted by applicable law. Prescriptions are not guaranteed. Bahari Medical providers do not prescribe controlled substances.
Plans that include the Prescription Discount Program provide access to 1,000+ medications available free at participating local pharmacies when prescribed.
Privacy During Telehealth Visits
Your health information and the content of your telehealth visits are handled by our partner platforms under their HIPAA-compliant privacy practices. Bahari Medical does not have access to the content of your clinical visits or your protected health information.
You are responsible for ensuring a private environment on your end during telehealth visits to protect your own confidentiality.
